Send us the name of your city, county, town, village, business, media, organization, church, or school, along with your area's specific emergency phone numbers for the back cover, and we will format a "Crisis Checklist" especially for you.  You then have three options: 1) We can send you a "hard copy" via Priority Mail, 2) Attach it in a private email to you, or 3) Post it below in Adobe® PDF (72KB) format.  You can print out as many copies as you need or send it electronically to a printer so you can reproduce them in volume.  Your Customized "Crisis Checklist" belongs to you.

1.  Click on Customized "Crisis Checklist" which will take you to our form. Fill in your area's specific information and click "submit."

 

2.  Choose your category from the list below: city, county, town, village, business, media, organization, church, or school and send us your payment.  We accept checks, money orders, and bank/cashier's check.  Within five business days after we receive your payment, you will receive— by email or, if you prefer, fax— your customized "Crisis Checklist" in Adobe® PDF format.

 

3.  Send back any changes or your approval (via email or fax) and, once changes are made and you have approved, you will receive your Customized "Crisis Checklist" according to your preference: Priority Mail, email, or download your Customized "Crisis Checklist" from this page.
